Insurance plans seldom cover the full amount of a hospital bill and usually require precertification or second medical/surgical opinions prior to admission. Your insurance plan pays only for services described in your insurance contract, and you may owe a balance for portions not covered.
If you have insurance, we recommend that you follow these guidelines prior to your appointment or hospital visit:
• Review your contract and contact your insurance agent or employer with any questions concerning your coverage before registration.
• Bring your identification and insurance card with you or have someone bring the forms necessary for individual coverage.
• If your insurance requires a deductible or co-payment, be prepared to pay at registration;
• If precertification is required, verify that it has been completed by your physician's office prior to your arrival.
• If you have Medicare or Medicaid coverage, your claim will be processed in accordance with current regulations.
Your healthcare coverage is a contract between you and your insurance company. USA Health cooperates to the fullest in expediting your claim. You are ultimately responsible for your account. Final payment or assignment of sufficient insurance benefits is required at the time of discharge. We accept American Express, Discover, MasterCard and Visa.
